The FDA label's weight/INR-tiered dosing was established in the trials that supported Kcentra's approval, but the delay of waiting on an INR result in a true emergency, along with cost and preparation time at higher doses, has driven interest in fixed low-dose regimens (typically 1500–2000 units regardless of weight or INR).
Randomized and observational comparisons of fixed-dose vs. weight/INR-based 4F-PCC for VKA-associated bleeding, including intracranial hemorrhage, have generally found comparable hemostatic efficacy and faster door-to-treatment time with fixed dosing. Individual trials remain underpowered to fully rule out differences in mortality or thromboembolic events, and results have varied enough across studies that this is not yet a settled question.
Current consensus, reflecting the Neurocritical Care Society/SCCM 2016 ICH reversal guideline and more recent editorial commentary on the fixed-dose trials, still favors weight/INR-based dosing as the preferred strategy. A fixed-dose-first regimen (roughly 1500–2000 units) is considered an acceptable alternative, primarily in the intracranial hemorrhage setting, provided a post-dose INR is checked 15–60 minutes after the infusion and a predefined supplemental/escalation dose is available if the target INR isn't met. Notably, reversal failure in these studies tracks more closely with baseline INR than with body weight, an important consideration for where an institution sets its escalation trigger.
Idarucizumab remains the specific reversal agent for dabigatran. For the factor Xa inhibitors (apixaban, rivaroxaban, edoxaban), andexanet alfa was voluntarily withdrawn from the US market effective December 22, 2025, after the ANNEXA-I trial showed a net increase in thromboembolic events, including MI, versus usual care in intracranial hemorrhage. 4F-PCC (25–50 units/kg, off-label) is now the practical default for factor Xa inhibitor-associated major bleeding.
