This narrative review synthesizes evidence positioning GLP-1 and dual incretin agonists as an intermediate step before revisional surgery for weight recurrence, guiding pharmacists on agent selection and patient stratification.
Weight recurrence following bariatric surgery poses a significant challenge due to the chronic nature of obesity. Incretin-based therapies, like glucagon-like peptide-1 receptor agonists (GLP-1 RAs) and tirzepatide, show effective weight loss post-surgery. These therapies offer viable alternatives or adjuncts to revisional surgery, with reduced complication risks. Incretin-based therapies can be key in managing post-surgical weight recurrence and may reduce the need for more invasive revisional procedures.
