Pharmacist-led transitions can reduce medication harm post-ED discharge.

Transitions from the emergency department (ED) to home are high-risk periods for medication-related harm, especially in older adults. A pharmacist-led transition-of-care program was co-designed, emphasizing discharge medication reconciliation, prescription review, and patient follow-up, deemed highly feasible with an 83% mean rating. The study outlines a feasible intervention for improving medication safety during transitions, but further testing in a pilot randomized controlled trial is needed. Co-designed pharmacist-led interventions may enhance medication safety during ED transitions for older adults.
