Octreotide is increasingly used off-label for gastrointestinal (GI) bleeding due to portal hypertension in children, but evidence-based guidelines are lacking. The review included 33 children; octreotide reduced bleeding episodes and transfusion needs in some cases, with mild and reversible hyperglycemia as the only reported side effect. Due to high risk of bias and heterogeneity in data, current evidence is very low certainty; more research is needed to confirm efficacy and safety for long-term use. Octreotide could be promising for pediatric GI bleeding due to portal hypertension, but its role needs further validation.
