Selective digestive decontamination (SDD) is used to prevent infections in ICU patients, but its impact on antibiotic resistance is not fully understood. SDD did not significantly change the E. coli pangenome, but it increased the prevalence of a tobramycin resistance gene in E. coli. Though SDD is effective for patient outcomes, it may contribute to resistance gene prevalence, necessitating cautious use in ICUs. SDD impacts antibiotic resistance gene prevalence but not the overall E. coli pangenome.
