KRAS Inhibitors May Unlock Immunotherapy in Pancreatic Cancer
This narrative review synthesizes preclinical and early clinical evidence that RAS inhibition remodels the immune microenvironment, offering oncology pharmacists a mechanistic framework for the coming wave of KRAS inhibitor plus immunotherapy trials in pancreatic ductal adenocarcinoma.

KRAS inhibitors may transform the treatment of pancreatic ductal adenocarcinoma (PDAC) by enhancing the efficacy of immunotherapy, which is currently ineffective in this setting. Preclinical studies show KRAS inhibitors reverse immunosuppression and facilitate T cell infiltration, with enhanced effects when combined with immune checkpoint inhibitors. This study suggests potential new treatment approaches for PDAC involving KRAS inhibitors and immunotherapy combinations, pending further clinical research. KRAS inhibitors could enhance immunotherapy effectiveness in PDAC by altering the tumor microenvironment.
